The network is a group of clinicians - licensed nurses, social workers and care managers who are skilled in the field of care management and capable of upholding SeniorBridge standards and practices. Members are expected to meet specific criteria for training, experience, certification or licensing, and insurance.

The Application Process
Each practice and clinician will complete an information sheet. Once completed, all members of the practice will be vetted to ensure compliance with SeniorBridge standards. A contract will then be issued.

Once you are an Approved Provider, SeniorBridge will send you a packet of materials that identify you as a member of the SeniorBridge Care Management Network. We will work with you to maximize the advantages of the working relationship and create a productive partnership.
